Octo Games Registration and Waiver

Parental/Guardian Consent:

This section is required for participants under 18 years of age.

Event Activities and Associated Risks:

• Red Light, Green Light: A strategic race testing agility and reaction time. Risks include slips, trips, falls, and collisions with other participants.​

• Tug of War: A test of raw strength and teamwork. Potential risks involve rope burns, back strains, and falls due to sudden releases of tension.​

• Wall Ball Toss: A fast-paced, simple street or schoolyard game where players take turns hitting a ball against a wall, aiming to avoid drops and receive the fewest "strikes" or penalties

• Ships Across the Ocean: is a tag-based, outdoor or gym game where players acting as "ships" try to run from one end line to the other without being tagged by a "captain" in the middle.​

• Hang Challenge: Endurance and grip strength are key. Participants may experience muscle strains, blisters, or falls if grip is lost.​

• Hold Your Weight: A true test of mental and physical endurance. Possible risks include muscle fatigue, strains, and overexertion.​

• Obstacle Course: A fast-paced test of agility, speed, and mental strength. Risks encompass trips, falls, cuts, bruises, and potential sprains.​

Health Information:

Media Release: I hereby grant MIB permission to use photographs and videos taken during the Octo Games event for promotional purposes, including but not limited to social media, websites, and marketing materials.

Waiver and Release of Liability: I, the undersigned, acknowledge that participation in the Octo Games involves physical activities that carry inherent risks, including but not limited to injury, illness, and property damage. I voluntarily assume all risks associated with participation and release MIB, its officers, employees, volunteers, and affiliates from any and all liability to the fullest extent permitted by law.
